A palatal expander is a small, custom-fabricated appliance that fits over several top teeth toward the back of the mouth. It has two halves connected with a screw system in the middle. The expander is activated by turning the screw with a special key, applying gentle pressure on the jaw, gradually widening it. Expanders work best in children ages 7-10, as their jawbones are still developing and more responsive to gradual expansion. Palatal expansion creates additional room in the upper jaw resulting in a wider, more balanced smile.

- Crowding: Before a child’s adult teeth grow in, the orthodontist can determine if there will be enough space to accommodate them. By widening the upper jaw, space is created for the permanent teeth, allowing them to grow into more natural alignment. This can prevent the need for tooth extractions in the future.
- Impacted Teeth: A tooth that has not erupted and is blocked by other teeth can become impacted. We see this most commonly with canine (eye) teeth. Widening the upper jaw can make room for a tooth to
erupt into the proper position on its own, avoiding more difficult procedures later on.
- Crossbite: A crossbite occurs when the upper teeth sit inside the lower teeth when biting. An expander widens the upper jaw to fit properly outside of the lower jaw, creating a more balanced growth pattern. Correcting crossbites early can prevent crowding, tooth impaction, jaw asymmetry, gum recession, and abnormal tooth wear.
- Improved Breathing: In some cases, a narrow upper jaw can restrict nasal airflow, contributing to breathing difficulties, especially while sleeping. By widening the jaw, a palatal expander can open the airway, improving breathing and helping prevent issues like mouth breathing and even obstructive sleep apnea in the future.
Treatment with a palatal expander is generally smooth and painless. Patients typically feel slight pressure during each adjustment, but it’s mild and temporary. An over-the-counter pain reliever can be taken if necessary. Once the expansion phase is complete, the expander is left in for three-to-six months to allow the widened jaw structure to stabilize.
